Take our Health Risk Assessment!
Associates who complete both a biometric screening with your physician AND the Succeed Health Risk Assessment questionnaire prior to November 18th, 2011 will receive a $40 per month discount on their health coverage. It only takes 20-30 minutes to complete, and your personal health information is kept completely private, in keeping with state and federal laws.
