VitalCare Basic Disease Managment Programs

All members are eligible for VitalCare basic disease management programs if they meet the criteria for each specific disease management program. Eligible members automatically receive program information in the mail. If a member needs additional information or would like to "opt out" of any program, they may call our Quality Improvement Division toll free at 1-800-321-7947 ext. 3097 or local Temple phone number (254) 298-3097.

We have the following basic disease management programs available for members at no cost.

VitalCare Lifestyle Management Programs

A variety of lifestyle management programs are available by going to the Lifestyle Management Programs page. Disease Management Program participants are invited to take a Health Risk Appraisal (VitalCare Succeed®) which results in tailored and targeted information and tools regarding lifestyle management.

The VitalCare CareTM for Your Health module is tailored to help manage long-term health issues including:

  • How to manage common symptoms
  • Establishing a routine for better medication management
  • Sticking with your treatment plan
  • Coping with daily challenges and disruptions
  • Strategies for helping you deal with stress and worry
  • Practical tips for getting a good night's sleep
  • Getting the support you need from family and friends
  • Learning how to talk with your doctor to get the answers you need
  • Understanding how your pharmacist can become a key part of your treatment team
