Congestive Heart Failure Program

  • Members age 18 to 75 are eligible for the Congestive Heart Failure (CHF) Program.
  • Members receive educational information regarding CHF.
  • Members are provided an opportunity to request additional educational materials related to heart disease.
  • Members receive a tool to document and track blood pressure readings for easy reporting to their primary care physician.
