
Members age 18 to 75 are eligible for the Diabetes Program. Objectives are that members will obtain important exams including blood pressure, foot exam, eye exam with ophthalmologist or optometrist and obtain follow-up care by primary care physician, including monitoring of medication and laboratory tests results (hemoglobin A1c, lipid profile and micro albumin). Other health conditions are considered such as hypertension and kidney complications.
